A 30-minute clinic consultation at Nuffield Health Plymouth Hospital and tailored dietary report 2nd clinic consultations for existing clients booking within 6 months of their last consultation. The follow-up consultation includes: 20 minutes assessment of your completed food and drink diary prior to our meeting 30 minutes clinic consultation discussing your individual progress with the diet plan 3 months email support – a scheduled interim email in-between appointments to review your progress and any queries. Dietary report highlighting progress to date and further recommendations and advice. After your consultation, your report will be prepared and you will receive a copy of this by secure email within 2 working days. When you book… After you have booked your session, you will be sent some forms to complete via a secure portal including a health questionnaire, food and drink diary and clinic agreement form, within 2 working days. This allows me to assess your health and dietary needs prior to your appointment. Terms Book the first session online and subsequent sessions can be arranged afterwards. Please note this appointment must be taken within 6 months of the booking date, or as agreed in advance with Lucy Pritchard. Cancellation Policy For cancellations, please contact me at least 48 hours prior to your appointment to avoid a late cancellation fee. I will always do my best to accommodate last minute rescheduling of appointments.
