A 1-hour online Zoom consultation and tailored dietary report Suitable for general advice that may not need follow up The fee reflects the time spent researching your health concerns and diet history prior to your appointment, a professional, friendly video consultation via Zoom or telephone (if preferred), together with report preparation, a clinic letter to your GP or other healthcare provider The initial consultation includes: 30 minutes assessment of your completed forms including a health questionnaire and food and drink diary prior to our meeting 1 x 60 minute Zoom consultation discussing your individual needs, any medical symptoms and medical history, eating habit and lifestyle factors and diet A full report of findings including a dietary report. The report will highlight: - Dietary analysis: suspected dietary intolerances and/or nutritional imbalances - Action points: suggested steps to be taken to correct/enhance your diet - Personalised diet plan: based on your eating habits, food preferences and medical needs After your consultation, your report will be prepared and you will receive a copy of this by secure email within 2 working days. When booking online, please allow at least 3 days in order to complete your food diary and health form in advance. Please call me if you require any further information or would prefer a telephone appointment. When you book… After you have booked your session, you will be sent a Zoom link and some forms to complete via a secure portal including a health questionnaire, food and drink diary and clinic agreement form, within 2 working days. This allows me to assess your health and dietary needs prior to your appointment. Terms Book the first session online and subsequent sessions can be arranged following our initial consultation. Please note this appointment must be taken within 6 months of the booking date, or as agreed in advance with Lucy Pritchard. Payment will be made in full in advance of our first consultation. Booking on an initial or a follow up booking does not enter you into an email support package, unless a package has been purchased. Up to 2 email queries are accepted in-between appointments. Cancellation Policy For cancellations, please contact me at least 48 hours prior to your appointment to avoid a late cancellation fee. I will always do my best to accommodate last minute rescheduling of appointments.
