Includes 1 in-person consultation at Nuffield Health, Plymouth Hospital and 2 online follow up consultations via Zoom, tailored dietary reports, educational resources and email support This package is ideal for those who need help to manage conditions including IBS, diverticular disease, coeliac disease and other conditions which require dietary modifications in which a course of 3 consultations is recommended. It is also suitable for clients looking for help managing other conditions i.e. women's health, cholesterol who need follow up support. The fee reflects the time spent researching your health concerns and diet history prior to your appointment, 3 dietetic consultations, 1 in-person and 2 online Zoom video consultations or telephone follow ups (if preferred), together with report preparation, a clinic letter to your GP or other healthcare provider, and an interim email in-between each appointment during the course of your booked programme. The Gut Health Package includes: 30 minutes assessment of your completed forms including a health questionnaire and food and drink diary prior to our meeting 60 minutes in-person consultation discussing your individual needs, any medical symptoms and medical history, eating habit and lifestyle factors and diet 2 x 30-minute follow-up online Zoom video consultations 3 months email support – a scheduled interim email in-between appointments to review your progress and any queries. This is limited to 2 emails between appointments. All reports and dietary education resource sheets When booking online, please allow at least 3 days in order to complete your food diary and health form in advance Please call me if you require any further information or would prefer a telephone appointment When you book… After you have booked your session, you will be sent some forms to complete via a secure email link including a health questionnaire, food and drink diary and clinic agreement form, within 2 working days. This allows me to assess your health and dietary needs prior to your appointment. Terms Book the first session online and subsequent sessions will be booked during our consultations. Sessions must be taken within 6 months unless otherwise agreed with Lucy Pritchard. Cancellation Policy For cancellations, please contact me at least 48 hours prior to your appointment to avoid a late cancellation fee. I will always do my best to accommodate last minute rescheduling of appointments.
